📢 OFFICIAL VERIFICATION AND APPEAL GUIDELINES:

To view the official roster or file a necessary clarification, please strictly adhere to the following instructions:

1️⃣ Roster Verification:

• Scan the QR Code provided to access and verify your name on the official list of academic awardees.

2️⃣ Academic Appeal Submission:

• Students who believe they qualify but are not included in the published list may formally file an appeal.
• Download the official Appeal Submission Form
• Important: All accomplished appeal forms must be printed and physically submitted to the Academic Office.

📅 DEADLINE FOR SUBMISSION: All printed appeal requirements must be submitted on or before Thursday, June 11, 2026, at 5:00 PM.

Note: Students are strongly encouraged to submit their documents as early as possible to ensure sufficient time for administrative evaluation and proper processing. Late submissions will no longer be entertained.
