
TES GRANTEES (BATCHES 1, 2, & 1.1)
2nd Semester AY 2025-2026
We are now calling for the submission of documentary requirements for our Continuing TES Grantees for the current term. This is in accordance with the revised TES guidelines (MC No. 03 s. 2024) to facilitate proper validation and verification by the UniFAST Regional Coordinators.

- Grantee Masterlist: Please check the attached file to confirm your status as an updated continuing grantee for this period.
- Submission Deadline: All documents must be submitted/uploaded no later than February 28, 2026 | 5:00 pm.
Required Documents
To ensure your validation is processed accurately, please provide the following (To be submitted via the designated Google Form):
- Filled-out Annex 3 (TES Continuing Form): To be submitted via the designated Google Form.
- Certified True Copy (CTC) of Certificate of Registration/Enrollment: Uploaded in PDF format.Note: Ensure your CoR is stamped as a “Certified True Copy” before uploading. For assistance with CTC, please coordinate with Ma’am Emy.
- ATM Account Information – Please provide your Account Number and a scanned photo of your ATM Card (ensure the account number and name are clearly visible).
- Additional Requirements (If Applicable) – PDF/photocopy of an HEI-approved Leave of Absence (LOA).
