TDP-TES GRANTEES (BATCH 3.1)
2nd Semester AY 2025-2026

We are now calling for the submission of documentary requirements for our Continuing TDP-TES Grantees for the current term. This is in accordance with the revised TDP-TES guidelines (MC No. 03 s. 2024) to facilitate proper validation and verification by the UniFAST Regional Coordinators.

  • Grantee Masterlist: Please check the attached file to confirm your status as an updated continuing grantee for this period.
  • Submission Deadline: All documents must be submitted/uploaded no later than February 23, 2026 | 5:00 pm.

Required Documents

To ensure your validation is processed accurately, please provide the following (To be submitted via the designated Google Form):

  1. Filled-out Annex 3 (TDP Continuing Form): To be submitted via the designated Google Form.
  2. Certified True Copy (CTC) of Certificate of Registration/Enrollment: Uploaded in PDF format.Note: Ensure your CoR is stamped as a “Certified True Copy” before uploading. For assistance with CTC, please coordinate with Ma’am Emy.
  3. ATM Account Information – Please provide your Account Number and a scanned photo of your ATM Card (ensure the account number and name are clearly visible).
  4. Additional Requirements (If Applicable) – PDF/photocopy of an HEI-approved Leave of Absence (LOA).
