Call for Submission of Documentary Requirements for CONTINUING TDP-TES GRANTEES(BATCH 3) – 1st Semester AY 2025-2026
Call for Submission of Documentary Requirements relative to the implementation of the TDP-TES stipulated in the MC No.03 s.2024 Revised TDP-TES guidelines, for the 2nd Semester of the Academic Year 2024-2025 Batch 3, for proper validation and verification of the UniFAST Regional Coordinators.
Attached is the masterlist of the updated Continuing TES-TDP grantees for 1st semester AY 2025-2026.
To ensure accurate validation and verification, the grantees are requested to submit/upload the following documentary requirements not later than Nov 12, 2025 | 3:00 PM:
Required Documents:
- 1). Filled-out Annex 3: TDP Continuing Form (via Google Form)
- 2). Copy of Certificate of Registration/ Certificates of Enrollment (PDF)( to be uploaded)
- 3). Additional Requirements (if applicable)
- Photocopy/pdf copy of Leave of Absence (LOA) approved by HEI
