To ensure the timely disbursement of stipends, we kindly request the following documents be submitted by August 26, 2025 | 5:00 p.m.:

1. Signed General Payroll Form: Please ensure the signature on the form matches the signature on your government-issued ID; signing on the payroll will start from September 2 to 5, 2025.

2. Certification Enrollment: Complete the online Certification Enrollment form for the following Semesters:
   BATCH 9.1: 2nd Semester SY 2023-2024 (2023-24B) – Fill out here
3. Landbank Personal Savings Account: Please submit your Landbank Personal Savings Account details through the Google Form, fill out here.
4. Photo of Active Landbank ATM Card: Upload a clear picture of your active Landbank ATM card through the Google Form, fill out here.
5. Photocopy of ID: Please submit four clear, double-sided photocopies of your government-issued ID, each with three blue specimen signatures. Additionally, a scanned copy of the ID must be uploaded through the Google Form link provided in this post, fill out here.
