Submission of Application for Graduation Documents

For Graduating Students Mid-Year 2024-2025

The submission of graduation documents will be from Feb 7-12, 2025, at Room 210.

Required Documents

• Two (2) copies of Application for Graduation
• Two (2) copies of 1st Evaluation Result
• One (1) copy of SIS-generated Curriculum/Grade Evaluation

All documents must be printed in “8.5 x 13”-size bond paper.
