To ensure the timely disbursement of stipends, we kindly request the following documents be submitted by July 26, 2024:

1. Signed General Payroll Form: Please ensure the signature on the form matches the signature on your government-issued ID.

2. Certification Enrollment: Complete the online Certification Enrollment form for the following Semesters:
   1st Semester SY 2022-2023 (2022-23A) – Fill out here
   2nd Semester SY 2022-2023 (2022-23B) – Fill out here
   1st Semester SY 2023-2024 (2023-24A) – Fill out here
3. Landbank Personal Savings Account: Please submit your Landbank Personal Savings Account details through the Google Form, fill out here.
4. Photo of Active Landbank ATM Card: Upload a clear picture of your active Landbank ATM card through the Google Form, fill out here.
5. Photocopy of ID: Provide four (4) clear, back-to-back photocopies of your government-issued ID with three (3) blue specimen signatures.

6. Contact Information: Submit a spreadsheet (Excel format) containing the updated email addresses and contact numbers of all grantees for Client Satisfaction Survey purposes.
