Submission of Application for Graduation Documents
For Graduating Students Mid-Year 2023-2024
The submission of graduation documents will be from January 4-12, 2024, at Room 202.
Required Documents
- Two (2) copies of Application for Graduation
- Two (2) copies of 1st Evaluation Result
- One (1) copy of SIS-generated Curriculum/Grade Evaluation
All documents must be printed in “8.5 x 13”-size bond paper.
