TDP-TES – UniFAST – Validation
In line with the Implementation of the Tulong-Dunong Program TES (TDP-TES) handled by the UniFAST Secretariat and for proper Validation and Verification of the UniFAST Regional Coordinators, Continuing TDP-TES grantees for AY 2023-2024 are requested to submit the following documentary requirements and provide needed information relative to the UniFAST Annex 5 forms until November 14, 2023 by filling out the google form by clicking “🔴(fill-out)” on the list below:
List of requirements for validation for 1st Semester A.Y. 2023-2024
1. Annex 5 ( Form 2 – Google Form link 👇):
2. Certificate of Registration (1st Semester A.Y. 2023-2024 – Students are required to upload a PDF copy of their COR, which is accessible and downloadable through their SIS account.)
Note: Only currently enrolled students are required to complete this form. If you are not enrolled or have graduated, please disregard this post.
