PARTIAL REQUIREMENTS includes:

• Original with dry seal or certified true copy with dry seal of Grade 10 & 11 High School Card
• PUP Medical Health Information Sheet for Student
• Two (2) pcs. of 2 x 2 inches colored picture with white background and with the applicant’s name on the photo
• One (1) pc. long brown envelope
• Two (2) pcs. expandable brown envelope
• SAR FORM
• CAEPUP Qualifier’s Notification

Completion of requirements is until August 31, 2022.

Confirmation of slot for all qualified applicants is also extended until July 4, 2022. Failure to confirm means forfeiture of your slot.
