ONLINE FILING AND PROCESSING OF ID APPLICATION
This ID application is intended only for (1st time/no ID application) students admitted in Academic Year 2024-25 only. (For lost, replacement, transfer and shiftee please send an email to Student Services at registrar.starosa@pup.edu.ph
- STEP 1 – Prepare your current Certificate of Registration (CoR). (download CoR here)
- STEP 2 – Download and fill out ID Application Form. (download form here)
- STEP 3 – Upload ID Application Form via Google Form. Continue
- STEP 4 – Submit the printed requirements (ID Application Form & CoR) to the Student Services office for verification and processing, submission of printed requirements starts on April 25 (to minimize crowding, we recommend per section submission of printed requirements through the section president or representative).
