🔎 HOW TO CONFIRM YOUR SLOT (STEP-BY-STEP GUIDE)

STEP 1: Find Your Name on the List

• 💻 Desktop Users: Press Ctrl + F on your keyboard to open the search bar.
• 📱 Mobile Users: Tap your browser’s menu (three dots ... or arrow icon) and select “Find in Page” or “Hanapin sa Pahina”.
• Type your Last Name or First Name in the search box to locate your record.

STEP 2: Click to Confirm

• Once you see your name, CLICK DIRECTLY ON YOUR NAME.
• It will automatically open your personalized, pre-filled Google Form.

STEP 3: Submit the Form

• Review the pre-filled information, complete the remaining fields, and hit Submit.
• Note: Please allow 5-10 minutes for your system confirmation notification to reflect.
