Submission of 1st Evaluation Results and Curriculum/Grade Evaluation For Graduating Students Year-End 2024-2025

The submission of graduation documents will be from June 17-20, 2025 at Room 202-Student Record Section.

Required Documents

  • One (1) copy of 1st Evaluation Result
  • One (1) copy of SIS-generated Curriculum/Grade Evaluation

All documents must be printed in “8.5 x 13”-size bond paper.
